COMPLIANCE WITH APPROVED PLANS <br /> <br />A. The property owner shall comply with Exhibits A thru D, dated January 15, 2004, <br />except as hereinafter modified. (Copies of Exhibit A thru D are on file in the <br />Community Development Department, 835 East 14th Street, San Leandro, <br />Califomia). <br /> <br />Exhibit A - Vicinity Map <br />Exhibit B - Aerial Photograph/Site Plan <br />Exhibit C - Floor Plan, 2nd Floor of Existing Medical Office Building <br />Exhibit D - Photographs of Subject Property <br /> <br />B. The applicant and/or property owner shall be responsible for assuring that any <br />successor in interest who assumes responsibility for this conditional use permit <br />approval is informed of its terms and conditions. <br /> <br />II. PERMITTED USE <br /> <br />A. This conditional use permit approval shall be for a massage therapy business in an <br />existing professional medical office building. A maximum of seven massage <br />therapy rooms are permitted. The hours of operation for the business shall be 9am <br />to 10pm, seven days per week. <br /> <br />III. MAINTENANCE <br /> <br />A. Any signage for this business shall be in keeping with the existing sign program for <br />this building. <br /> <br />IV. GENERAL REQUIREMENTS <br /> <br />A. All clients shall be seen at the office at 215 Estudillo Avenue; out-call massage <br />visits are prohibited by Municipal Code Section 4-23-540. <br /> <br />B. All employees of this business or contractors with the business involved in massage <br />of clients shall be Certified Massage Therapists (CMTs) and shall obtain individual <br />massage therapist permits in compliance with Municipal Code Section 4-23-400. <br /> <br />Agreement to Conditions <br />PLN2003-00116 <br /> <br />January 15, 2004 <br />Page 3 of 4 <br />
