<br />The City will contribute the following monthly amount, including the PERS Medical <br />Plan "employer minimum share" contribution, towards the election of medical benefits in <br />the plan. <br /> <br />Employee <br />Employee + 1 <br />Family <br /> <br />2006 <br />$ 389.38 <br />$ 778.76 <br />$1,012.39 <br /> <br />In 2006, employees electing to enroll in the PERS Medical Plans will be required to <br />contribute the difference between the above amounts and the actual cost of the medical <br />plan premium. Such employee contribution will be on a pre-tax basis as specified above. <br /> <br />In 2007, employees shall continue to contribute the difference between Kaiser Bay <br />Area/Sacramento (BA/S) rate and any alternate elected premium. In addition, all <br />employees will contribute 40% of the K/BA premium increase over the 2006 rates to a <br />maximum of the following amounts: <br /> <br />Employee <br />Employee + 1 <br />Family <br /> <br />$lO/month <br />$25/month <br />$50/month <br /> <br />In 2008, employees shall continue to contribute the difference between Kaiser BA/S rate <br />and any alternate elected premium. In addition, all employees will contribute 40% of the <br />K1BA premium increase over the 2007 rates to a cumulative maximum of the following <br />amounts: <br /> <br />Employee <br />Employee + 1 <br />Family <br /> <br />$20/month <br />$40/month <br />$80/month <br /> <br />Employees who wish to waive enrollment in the medical and dental plans and who <br />demonstrate to the satisfaction of the City their enrollment in another medical and dental <br />plan, shall receive one hundred and fifty dollars ($150.00) per month for their non- <br />election of medical and dental benefits. In the event both spouses are employed by the <br />City and eligible to enroll in the City's flex medical benefits plan, one employee may <br />elect not to enroll in the medical and dental plans and will receive the one hundred and <br />fifty dollars ($150.00) per month payment, if enrolled under spouse's coverage. <br /> <br />An employee may elect non-enrollment in the medical plan and enrollment in the dental <br />plan and receive one hundred dollars ($100.00) per month for their non-election. <br /> <br />The above opt-out premiums will increase to two hundred dollars ($200.00) per month <br />and one hundred and fifty dollars ($150.00) per month, respectively, effective January 1, <br />2007. <br /> <br />slmo.OS-OI-06 JC <br /> <br />2 <br />
