The San Leandro Shelter will provide emergency food and housing to homeless women and children. The <br />availability of a safe, stable, and supportive living environment, with access to quality support services will <br />increase the probability of improving their ability to become self-sufficient. <br /> <br />B. The Shelter will provide one-on-one case management to homeless women in support of their housing and <br />employment searches. This would increase their probability of securing housing and/or employment. <br /> <br />MAJOR ACTIVITIES and PERFORMANCE MEASURES <br /> <br />SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT: <br /> <br />1. Provide 24-hour emergency housing to 250-300 homeless women and children (794 bed nights), three <br />meals a day, access to health care, parenting and recovery support groups, domestic violence and self-esteem <br />building support groups, and money management. Eighty-five percent (85%), or 58 of the 68 women who stay <br />30 days or longer, will increase their "level of self-sufficiency" by one level in one or more of the following <br />areas: housing, employmenUincome, domestic violence, substance abuse, mental health, and/or physical <br />health. * <br /> <br />2. Provide one-on-one case management (support services) to 80-100 women, which include, working <br />with housing and employment specialists. Sixty percent (60%), or 41 of the 68 women staying for 30 days or <br />more, will leave the shelter with long-term housing and/or employment. <br /> <br />REPORTS AND MISCELLANEOUS <br /> <br />1. For CDBG funds, Agency must submit quarterly and annual reports using the City's reporting form <br />within 15 days of the end of the quarter on the number of persons served. Quarterly reports shall include a <br />narrative section and shall be submitted even if there are no specific numbers to report.
