Objectives - Program Outputs and Outcomes <br />Please submit no more than two Objectives. In many cases, only one Objective may be sufficient. See Page 14 for Example of <br />Completed Proqram Objectives Form. <br /> <br />I Objective #1. <br /> <br />I <br /> <br />a. Program Premise (if..., then...): <br />!! unemployed and underemployed families, experiencing a life/financial crisis, have access to comprehensive support <br />services such as food, clothing, information and referral, nutrition education, motel vouchers, PG&E assistance, health care, <br />and linkages to other resources such as affordable quality childcare ~ their risk of compromising their self-sufficiency will <br />\:II:> rpduced. <br /> <br />b. Program Activities: (Describe the services provided) <br />o A week's worth of groceries which includes: canned goods, bread, produce, as well as dairy and meat products made ! <br />available through our new acquisition of refrigeration <br />o Clothing/household items <br />o Emergency motel vouchers <br />o PG&E Utility assistance <br />o Nutrition education and life skills training designed to teach participants how to budget and shop with limited resources <br />and fixed incomes <br />o Information & referral to services not provided such as rental assistance, shelter, etc. <br />Cl Job preparation and placement services which includes computer classes, soft skills training, resume writing and <br />interviewing skills <br />o Internal linkages to counseling and child care <br />o Free acute medical care and enrollment and referrals to affordable health care plans <br /> <br />c. Program Outputs (The direct quantifiable products of program activities, such as the number of meals provided or <br />participants served. Program outputs should be measurable and realistic): <br /> <br />BY JUNE 30. 2007 (12 MONTHS): <br /> <br />500 families will be served each month totaling 6000 families served within twelve months. The number of meals provided per <br />month is 31,500 totally 378,000 within twelve months, <br /> <br />BY JUNE 30. 2008 (24 MONTHS): <br /> <br />1000 families will be served each month totaling 12,000 families served within twelve months, The number of meals provided per <br />month is 63,000 totally 756,000 within twelve months. <br /> <br />d. Program Outcomes (Program Outcomes = The benefit for program participants. Who benefits,"what will change, by when, how will <br />vou know.): <br />BY JUNE 30, 2007 (12 MONTHS): <br /> <br />40% of the low-income families who receive comprehensive support services will reduce their risk of losing their job by <br />maintaining employment during the first month following their crisis or use of services. <br /> <br />BY JUNE 30. 2008 (24 MONTHS): <br /> <br />40% of the low-income families who receive comprehensive support services will reduce their risk of losing their job by <br />maintaining employment during the first month following their crisis or use of services. <br />
