Provide one-on-one case management support services to 20-25 women. 60% or 10 of the 17 women who <br />stay at the shelter 30 days or more will depart the shelter with safe, stable housing. <br />By January 1, 2009, Building Futures will have accomplished: <br />Provide 24-hour emergency housing to 125-150 homeless women and children (406 bed nights), three meals a <br />day, access to health care, parenting and recovery support groups, domestic violence and self-esteem building <br />support groups, and money management. 75% or 26 of the 34 women who stay at the shelter 30 days or <br />more will address two or more of the specific barriers they face to achieving safe and stable housing. <br />Provide one-on-one case management support services to 40-50 women. 60% or 20 of the 34 women who <br />stay at the shelter 30 days or mare will depart the shelter with safe, stable housing. <br />By April 1, 2009, Building Futures will have accomplished: <br />Provide 24-hour emergency housing to 188-225 homeless women and children (608 bed nights), three meals a <br />day, access to health care, parenting and recovery support groups, domestic violence and self-esteem building <br />support groups, and money management. 75% or 38 of the 51 women who stay at the shelter 30 days or <br />more will address two or more of the specific barriers they face to achieving safe and stable housing. <br />Provide one-on-one case management support services to 60-75 women. 60% or 31 of the 51 women who <br />stay at the shelter 30 days or more will depart the shelter with safe, stable housing. <br />By July 1, 2009, Building Futures will have accomplished: <br />Provide 24-hour emergency housing to 250-300 homeless women and children (811 bed nights), three meals a <br />day, access to health care, parenting and recovery support groups, domestic violence and self-esteem building <br />support groups, and money management. 75% or 51 of the 68 women who stay at the shelter 30 days or <br />more will address two or more of the specific barriers they face to achieving safe and stable housing. <br />Provide one-on-one case management support services to 80-100 women. 60% or 41 of the 68 women who <br />stay at the shelter 30 days or more will depart the shelter with safe, stable housing. <br />REPORTS AND MISCELLANEOUS <br />1. For CDBG funds, Agency must submit quarterly and annual reports using the City's reporting form <br />within 15 days of the end of the quarter on the number of persons served. Quarterly reports shall include a <br />narrative section and shall be submitted even if there are no specific numbers to report. The narrative report <br />should describe why no specific numbers are reported. <br />2. To ensure timely expenditure of HUD funds, Agency must submit monthly reimbursement claims <br />using the City's invoice or comparable form with documentation of claim including copies of time sheets <br />(denoting specific hours for CDBG work performed), payroll stubs, DE3 or employment or contract letter for <br />personnel costs, invoices or billings. <br />3. For Human Services funds, if applicable, Agency must submit semi-annual activity reports in the format <br />required by the City. <br />4.
