SECTION I <br />GUIDELINES FOR THE DISTRIBUTION OF COMMUNITY <br />ASSISTANCE FUNDS <br />MISSION STATEMENT <br />Through the Community Assistance Grant Program, the City of San Leandro supports <br />organizations that offer programs designed to foster the independence of individuals and families <br />and prevent the need for future services. The City also supports programs that assist persons in <br />crisis and those who lack the basic necessities of life. Funded agencies must provide affordable <br />services to individuals and families living in San Leandro. The Human Services Commission <br />will review each proposal and assess the city's need for that program and make funding <br />recommendations to the City Council. <br />Continuum of Need <br />The goal of the Community Assistance Grant Program is reflected in the "Continuum of Need" <br />funding guidelines described below. These policies and guidelines are based on a model, which <br />categorizes services along a "continuum of need." Each category represents different types of <br />services that are designed to prevent an individual from moving across the continuum to a <br />greater level of need. Community Assistance funding places emphasis on enabling and <br />prevention services while recognizing the need to support basic services. <br />I. ENABLING AND PREVENTION SERVICES <br />Services that are designed to foster independence and/or prevent people from needing <br />social services in the future. <br />II. CRISIS INTERVENTION SERVICES <br />Services that help people who are in crisis. <br />III.
