Appendix A <br />Scope of Work <br />The following Scope of Work is agreed to by the Alameda County Waste Management Authority <br />(Grantor) and the City of San Leandro (Grantee). <br />Proiect Description <br />San Leandro Senior Community Center <br />The City of San Leandro is building a new community center for seniors on East 14th Street. <br />The project is required by the City's ordinance to achieve at least LEED-Silver certification in the <br />LEED for New Construction Rating System. Currently the project is tracking 41 points. A <br />minimum of 39 points must be achieved for Gold, but often projects will lose 1-3 points during <br />construction due to change orders or unforeseen conditions. Therefore, this grant will include a <br />provision for a higher grant amount should the project achieve LEED-Gold. <br />Outside, the project is following Bay-Friendly Landscaping practices and is expected to score 91 <br />points through the Bay-Friendly Landscaping program. <br />Project Roles & Responsibilities <br />City of San Leandro Key Personnel <br />Mark Goralka, Associate Engineer; Uche Udemezue, Engineering & Transportation <br />Department Director. <br />The City of San Leandro will: <br />1. Monitor the contracted design and construction professionals' work to ensure that the <br />design and construction of the Senior Community Center meets the LEED Silver <br />standard (or greater) and the minimum requirements and score (or greater) on the Bay- <br />Friendly scorecard and complies with the terms of the Authority Green Building & Bay- <br />Friendly Landscaping Grant. <br />2. Divert at least 75% of all construction and demolition (C&D) debris as included in project <br />specifications, design drawings, and LEED checklist. <br />3. Communicate questions regarding green building and Bay-Friendly Landscaping <br />practices and the use of Green Building and Bay-Friendly Landscape Grant funds to the <br />appropriate Authority project manager in an expeditious manner. <br />4. Submit copies of all publicity or promotional materials concerning the project, including <br />press releases, feature stories, public services announcements and brochures, to the <br />Authority Program Manager for review and approval prior to release of the information. <br />5. Notify the appropriate Authority Program Manager in a timely manner of any changes to <br />the project that will result in the Grantee's inability to fulfill the terms of the contract. <br />2.
