That on or about the 3rd day of October 2007, the City of San Leandro, a municipal corporation of the State of California having offices at 835 East 14th Street, San Leandro, California, did enter into a certain contract with O. G JONES & SONS, INC., 1520 FOURTH STREET, BERKELEY, CA 947,1.0, as contractor, for a certain work of improvement: to wit SAN LEANDRO SENIOR CENTER -PHASE 1, PRO.IECT NO. 06-210-1 S-I l fi. 2. That the name of the surety upon the bond of the Contractor for said work of improvement is as follows: Liberty Mutual Insurance Company. 3. That said work of improvement was accepted by the City Council of the City of San Leandro as completed on the 17r~ day of February 2009. 4. The nature of the interest or estate of the City of San Leandro as constructor of the SAN LEANDRO SENIOR CENTER- PHASE 1, PROJECT NO. 06-210-18-116 project is that of improvements made to certain private land(s) designated as 13889 East 14r~ Street, APN: 077D-1429-003-05. This parcel is owned by Eden Healthcare Township District and has a permanent public ingress/egress and parking easement. I certify under penalty of perjury that the foregoing is true and correct.
