passenger overloads and pass ups, schedule adherence problems, and any discrepancies with trip <br />information provided on the driver manifests. <br />Drivers will update all hardcopy manifests as directed by the dispatcher, recording all <br />cancellations, no-shows, add-ons, and walk-on boardings by designated bus stop_ Separate <br />manifests will be maintained for each individual scheduled run. <br />Drivers will be responsible for the collection and safeguard of all fares and/or fare medium. <br />CONTRACTOR and City agree that during the term of this Agreement, and for a period of one <br />year following the termination thereof, neither party shall hire an employee or former employee <br />of the other party without the written approval of the other party. The term "former employee" <br />used in this provision shall mean employees who have separated their employment within the <br />prior twelve months. <br />2.10. DATA COLLECTION AND REPORTING <br />The CONTRACTOR wi-11 collect and maintain all operating and performance data required by <br />the CITY. All reports and/or background data will be certified by the CONTRACTOR to be <br />accurate. <br />The CONTRACTOR shall collect and maintain completed daily driver manifests and dispatch <br />logs. These will provide the base data for the monthly and annual performance and operating <br />reports, fare box reconciliation, and monthly CONTRACTOR billings. <br />The CONTRACTOR will maintain a set of completed, readable driver manifests and dispatch <br />logs for a three (3) year period, and upon request be made available to the CITY for planning and <br />auditing purposes. <br />The CONTRACTOR shall prepare monthly management reports to include daily ridership totals <br />by route, as well as monthly breakdowns of: boardings by ambulatory passengers and passengers <br />using wheelchairs, attendant trips, walk-on boardings by designated bus stop, and request zone; <br />total trip requests, cancellations, no-shows (passenger & contractor), late pick-ups and completed <br />trips; eligible passengers and companions; cash fares and tickets; total revenue hours and miles <br />operated; complaints; and the number of vehicle breakdowns and accidents. CONTRACTOR <br />may be asked to collect further data as required by ACTIA. CONTRACTOR will be asked to <br />collect and report data in format as required by ACTIA. <br />The CONTRACTOR shall report all in-service incidents involving property damage or personal <br />injury accidents, illegal or threatening actions, and unacceptable passenger and/or staff behavior <br />to the CITY immediately upon knowledge of the situation. <br />EXHIBIT A Page 10 of 13 <br />G:\Oliver\ParatransitFY 2004-05\\MV Contract FY 2004-OS <br />
