• Provide current billing expenses, as well as a running total of expenses, payments received, invoices billed, and remaining balances on each purchase order. • Provide an itemized invoice listing of billed reimbursable expenses, as well as a running total of reimbursable expenses • Reference project account number (961-86-045) as well as the purchase order number as designated 2.5 Miscellaneous • Unless specifically noted otherwise, all meetings with City Staff shall occur in San Leandro City Hall located at 835 East 14th Street in San Leandro during normal business hours. 3.0 PROJECT SPECIFIC SCOPE OF WORK AND DELIVERABLES 3.1 Develop Joint Trench Plan Contractor shall use the joint trench plan developed for the West Estudillo Pedestrian Connection project. Contractor shall update the West Estuillo Pedestrain Connection projects plan accordingly to be consistent with the requirements of the utility companies and completed "Form B" 3.2 Preparation of Utility Cross Sections Contractor shall prepare utility cross sections far each different joint trench condition. • Plans shall include applicable utility details and specifications. • Define all utility companies involved • Define the size and quantity of conduits each utility company desires Deliverable: Five (5) copies each to each participating agency . 3.3 Preparation of "Form B" Authorization Contractor to complete all "Form B" cost breakdown for all participating joint trench occupants. Costs shall be latest construction costs: • Define all utility companies involved • Define the size and quantity of conduits each utility company desires • Breakdown of costs with amounts attributable to all participating agencies. • Revise Form B estimate with actual bid costs and construction costs. Deliverable: Five (S) copies each to each participating agency . 3.4 Design Management and QA/QC Contractor shall provide overall project management and maintain effective communications with City staff and participating utilities throughout the duration of this phase. Contractor to review and revise documents as necessary to meet the requirements of the participating joint trench agencies and utilities.
