The cost of professional liabilit~~ insurance and all costs associated with cell phones, electronic mail, faxes, long distance phone charges and related telecommunications shall be charged as a combined surcharge of 2.5% on the total fees. Payments Payments are due within ten da~7s after montl~v billing with amounts more than thirty days past due subject to a 1.5 % per month interest charge.
