Homelessness Prevention and Re-housing 2 October 5, 2009 <br />Program Plan MOU <br />Family Resource Center, to submit an HPRP grant application for $1.5 million to State HCD, <br />which approved the grant request in late September 2009. <br />HPRP is focused on housing services for homeless individuals and families and those at-risk <br />of homelessness. HPRP funds will provide temporary financial assistance and housing <br />relocation and stabilization services to individuals and families who are homeless or would be <br />homeless without this assistance. The primary goal is to either prevent individuals and <br />families from becoming homeless or help those who are experiencing homelessness to be <br />quickly re-housed and stabilized. <br />In Alameda County, Everyone Home (EOH) is leading the effort to improve homeless and <br />homelessness prevention services by coordinating the implementation of a county-wide <br />Homelessness Prevention and Re-housing Program to simplify client accessibility, streamline <br />services, and provide common performance outcome data and measurable results. In 2006, <br />the City adopted EOH's countywide 15-year plan to end chronic homelessness. This plan <br />outlines a reorientation of housing and service systems to end chronic homelessness and <br />significantly reduce housing crises for vulnerable populations in Alameda County. One of <br />EOH's key strategies is to prevent homelessness and other housing crises. To coordinate <br />regional HPRP services, EOH has held numerous jurisdictional, service provider and <br />countywide community meetings to develop a homeless and homelessness prevention service <br />delivery system that is both effective and efficient for clients. <br />The county HPRP Plan focuses on the development of regional Housing Resource Centers <br />(HRC) that will provide a variety of homelessness prevention and re-housing services, linked <br />through a countywide referral, assessment and outcome tracking system. Each HRC will <br />provide a similar complement of services, including one-time, short- and medium-term <br />financial assistance, case management and housing locator services to prevent homelessness <br />and rapidly re-house those who have become homeless. Throughout the county, Housing <br />Resource Centers will provide the core HPRP and other social services in house and will also <br />provide links to other services not available at an HRC such as enrollment into mainstream <br />benefits programs. Housing Resource Centers are located in north county (Berkeley and two <br />HRCs in Oakland), east county (Livermore), south county (Fremont), and mid county (San <br />Leandro). <br />Eligible Activities <br />Eligible activities are specifically focused on housing: either financial assistance to help pay <br />for housing or services designed to keep people in housing or to find housing. The intent of <br />HPRP assistance is to transition program participants rapidly to stability, either through their <br />own means or appropriate public assistance. <br />HPRP funds may be used for services that assist program participants with housing stability <br />and placement. These services are limited to the following eligible activities: case <br />management; housing search and placement; credit repair; legal services; and, outreach and <br />engagement. HPRP assistance is not intended to provide long-term support for program <br />participants, nor will it be able to address all of the financial and supportive services needs of <br />households that affect housing stability.
