That on or about the 20th day of July 2009, the City of San Leandro, a municipal <br />corporation of the State of California having offices at 835 East 14th Street, San Leandro, <br />California, did enter into a certain contract with FBD VANGUARD <br />CONSTRUCTION, 651 ENTERPRISE COURT, LIVERMORE, CA 94550, <br />PHONE: 925-245-1300, as contractor, for a certain work of improvement: to wit <br />WICKS BOULEVARD SIDEWALK, PROJECT N0.08-144-36-287. <br />2. That the name of the surety upon the bond of the Contractor for said work of <br />improvement is as follows: ARCH INSURANCE COMPANY. <br />3. That said work of improvement was accepted by the City Council of the City of San <br />Leandro as completed on the 1St day of February, 2010. <br />4. The nature of the it?kerest or estate of the City of San Leandro as owner of the WICKS <br />BOULEVARD SIDEWALK, PROJECT NO. 08-144-36-287 project is that of public <br />improvements made within public right-of--way on Wicks Boulevard between Stenzel <br />Park and Burkhart Avenue, as well as ownership of improvements thereon. <br />I certify under penalty of perjury that the foregoing is true and correct.
