TIMELINES <br />By October 1, 2010, Davis Street will have accomplished: <br />Provide sixty-three (63) San Leandro clients with crisis intervention and/or short term community <br />counseling services and access to eligible family resource center services. <br />Seventy-five percent (75%) or forty-seven (47) of the sixty-three (63) San Leandro clients will report a <br />reduction in the severity or frequency of the presenting issue as supported by a decrease in high-risk <br />behavior. <br />By January 1, 2011, Davis Street will have accomplished: <br />• Provide 1.26 San Leandro clients with crisis intervention and/or short term community counseling <br />services and access to eligible family resource center services. <br />• Seventy-five percent (75%) or ninety-four (94) of the 126 San Leandro clients will report a reduction in <br />the severity or frequency of the presenting issue as supported by a decrease in high-risk behavior. <br />By April 1, 2011, Davis Street will have accomplished: <br />Provide 190 San Leandro clients with crisis intervention and/or short term community counseling <br />services and access to eligible family resource center services. <br />Seventy-five percent (75%) or 142 of the 190 San Leandro clients will report a reduction in the severity <br />or frequency of the presenting issue as supported by a decrease in high-risk behavior. <br />By July 1, 2011, Davis Street will have accomplished: <br />Provide 255 San Leandro clients with crisis intervention and/or short term community counseling <br />services and access to eligible family resource center services. <br />Seventy-five percent (75%) or 191 of the 255 San Leandro clients will report a reduction in the severity <br />or frequency of the presenting issue as supported by a decrease in high-risk behavior. <br />-14- <br />CDBG Agreement with DSFRC - <br />Community Counseling Program FY10-11 <br />
