<br />STP/CMAQ Application <br />Rehabilitation Supplement <br />Illtatllon Proaram 5creenlna criteria <br />Check all screening criteria that apply: <br />Project is eligible for STP or CMAQ under Federal guidelines and is consistent with the list of eligible strategies. <br />^ Project is in an adopted regional/local plan. <br />^ Project is consistent with the 1998 RTP. <br />^~ Project funds can be fully obligated by September 30, 2002. <br />Q Project is financially viable and fully funded. The non-federal match is at least 11.5% of the total project cost. <br />Project is well defined, and results in a usable segment. <br />^ If a streets and roads project, the sponsoring agency has a certified Pavement Management System. <br />^~ Project meets rehabilitation standards (please check appropriate standard below): <br />^ Roadway rehabilitation projects have a PCI of 60 or less. <br />^ Eligible preventive maintenance projects have a PCI of $5 or less. <br />^ Transit projects use/age consistent with FTA regulations. <br />Rehabilitation Program Supplemental Information <br />Project Type: Roadwa <br />Transit ~ <br />Roadwa ~ <br />Project Tier: Tier 2 -Non-MTS Pavement <br />Roadway <br />Tier 1 -MTS Pavement ~ <br />Tier 2 -Non-MTS Pavement ~ <br />MTS =Metropolitan Transportation System. <br />Non-MTS must be federal aid eligible -- includes road- <br />ways classified above that of rural minor collector or urban <br />local. <br />5% Percentage of Project Cost for Amenities (for Roadway Projects Only) <br />Amenities include bike lanes, signalization, turn pockets, transit pull-outs, sidewalk ramps, guard rails, and culverts.
