Goals-The projected goals, indicated numerically, and also the goals achieved (for each <br />report period). In addition, identify by percentage and description, the progress achieved <br />towards meeting the specified goals; .additionally identify any problems encountered in <br />meeting goals. <br />iv.) Beneficiaries- Provide the following: <br />1. Total number of direct beneficiaries. <br />2. Percent of total number of direct beneficiaries who are: <br />a) Low and moderate income <br />b) Low income <br />c) Black, not Hispanic origin <br />d) White, not Hispanic origin <br />d) Hispanic <br />e) American Indian/ Alaskan Native <br />f) Asian or Pacific Islander <br />g) Female Headed Households <br />v.) Other data as required by City. <br />b.
