2. Patient/Visitor Transportation Survey shall be conducted on the same <br />schedule as the employee survey by interviewing a representative sample <br />of patients/visitors, with the sample size being no less than 350 and <br />increasing with the increasing patient/visitor population, about their `travel <br />behavior on the day of the survey. The patient/visitor survey shall be <br />carried out at the same time as the employee survey is conducted, and shall <br />be subject to review and approval by the City. <br />3. Annual Process Evaluations: Kaiser shall report major accomplishments <br />achieved for and changes made to each of the measures in operation as <br />well as participation in each measure (e.g. number of participants in <br />Commuter Tax Incentive, Commuter Check Subsidy, carpool program) <br />and actual number of Full Time Equivalent staff (both am/pm peak and <br />non-peak). <br />IX. MAINTENANCE <br />A. The project site shall be well maintained and shall be kept free of litter, debris and <br />weeds at all times; during construction, the site shall be well maintained and shall <br />be kept free of litter, debris and weeds. <br />B. Any graffiti shall be promptly removed from building walls, perimeter soundwalls <br />and/or fences. The developer and its successors in interest shall comply with the <br />rules and regulations of the City's graffiti removal program and shall grant a <br />license and right of entry as requested to enforce the terms of such program. <br />C. All landscaping improvements shall be maintained in a healthy, growing <br />condition at all times. <br />D. Upon demolition or removal of existing structures and improvements, the site <br />shall be enclosed with a security fence and shall be well maintained in a neat <br />manner, free of weeds, litter and debris. <br />X. CONSTRUCTION PROVISIONS <br />A. Construction activity shall not commence prior to 7:00 a.m. and shall cease by <br />7:00 p.m. Monday through Friday, and construction activity shall not commence <br />prior to 8:00 a.m. and shall cease by 7:00 p.m. on Sunday and Saturday. No such <br />construction is permitted on Federal holidays. As provided in this City of San <br />Leandro's Noise Ordinance (ORDINANCE NO. 2003 - 005), "construction" <br />shall mean any site preparation, assembly, erection, substantial repair, alteration, <br />demolition or similar action, for or on any private property, public or private <br />right-of--way, streets, structures, utilities, facilities, or other similar property. <br />Construction activities carried on in violation of this Article may be enforced as <br />provided in Section 4-11-1130, and may also be enforced by issuance of a stop <br />work order and/or revocation of any or all permits issued for such construction <br />activity. <br />PLN2009-00030 Page 15 of 47 <br />Conditions of Approval May 3, 2010 <br />
