SECTION 6. PROGRAM MONITORING AND EVALUATION <br />a. Recipient shall be monitored and evaluated in terms of its effectiveness and timely <br />compliance with the provisions of this Regulatory Agreement and the effective and efficient <br />achievement of the program objectives. <br />b. Recipient must undertake continuous quantitative and qualitative evaluation of the <br />use of property as specified in this Regulatory Agreement and shall make annual written reports <br />to City. <br />1. The annual written reports shall include, but shall not be limited to the <br />following data elements: <br />activities/operations. <br />(i) Title of program, listing of components, description of <br />(ii) Service area (i.e., citywide, etc., including applicable census tracts). <br />(iii) Goals -the projected goals, indicated numerically, and also the goals <br />achieved (for each report period). In addition, identify by percentage and description, the <br />progress achieved towards meeting the specified goals; additionally, identify any problems <br />encountered in meeting goals. <br />(iv) Beneficiaries -provide the following: <br />a. Total number of direct beneficiaries <br />b. Percent of total number of direct beneficiaries who are: <br />- low and moderate income <br />- Black, not Hispanic origin <br />- White, not Hispanic origin <br />- Hispanic <br />- American Indian/Alaskan Native <br />- Asian or Pacific Islander <br />- female headed households <br />(v) Other data as required by City. <br />2. The annual report shall be due on the thirtieth day of July. <br />C. The City shall have ultimate responsibility for overall project monitoring and <br />evaluation to assist Recipient in complying with the scope and contents of this Regulatory <br />Agreement, and to provide management information which will assist the City's policy and <br />decision makers and managers. <br />3 <br />
