communication in advance of any change in volume or character of pollutants in their discharge, <br /> including the listed or characteristic hazardous wastes for which the User has submitted initial <br /> notification under local, Federal or State notification requirements. <br /> (c) All Users shall notify the Manager, the EPA Regional Waste Management <br /> Division Director, and State hazardous waste authorities in writing of any discharge into the <br /> POTW of a substance, which, if otherwise disposed of, would be a hazardous waste under 40 <br /> CFR Part 261. Such notification shall include the name of the hazardous waste as set forth in 40 <br /> CFR Part 261, the EPA hazardous number and the type of discharge (continuous, batch or other). <br /> If the discharge exceeds one hundred (100) kilograms of hazardous waste or any amount of acute <br /> hazardous waste in any calendar month the notification shall also contain the following <br /> information: An identification of the hazardous constituents contained in the wastes, the mass <br /> and concentration of such constituents in the wastestream discharged during that calendar month <br /> and the mass of the constituents in the wastestream expected to be discharged during the <br /> following twelve (12) months. In the case of any notification made under this paragraph, the <br /> User shall certify that it has a program in place to reduce the volume and toxicity of hazardous <br /> wastes generated. <br /> (d) Within 180 days after the effective date of a Categorical Pretreatment Standard or <br /> 180 days after the final administrative decision made upon a category determination submission <br /> under 40 CFR Part 403.6(a)(4), whichever is later, existing Users subject to such Categorical <br /> Pretreatment Standards and currently discharging to or scheduled to discharge to the POTW are <br /> required to submit to the Manager a Baseline Monitoring Report (BMR). At least 90 days prior <br /> to commencement of discharge, new sources, and sources that become Industrial Users <br /> subsequent to the promulgation of an applicable categorical standard are required to submit to <br /> the Manager a Baseline Monitoring Report (BMR). The BMR shall contain the information as <br /> required in 40 CFR Part 403.12(b). Part 403.12(b) of 40 CFR and any amendments thereto are <br /> incorporated by reference into this ordinance. <br /> Section 3 -14 -505. Permit Application. <br /> All major contributing industries and industries using/discharging EPA priority pollutants <br /> shall complete and file with the Manager, a permit application accompanied by the applicable <br /> fees. The applicant may be required to submit, in units and terms appropriate for evaluation, the <br /> following information, including, but not limited too: <br /> (a) Name, address and User classification number (SIC) of applicant; <br /> (b) Volume of wastewater to be discharged; <br /> (c) Wastewater constituents and characteristics including but not limited to those <br /> mentioned in these regulations as determined by a state certified laboratory; <br /> (d) Time and duration of discharge; <br /> (e) Average and maximum wastewater flow rates, including daily, monthly and <br /> seasonal variations, if any; <br /> 18 <br />
