3. Dependent Care Flexible Spending Account up to a $5,000 maximum per benefit plan year; <br /> 4. A "Core Flex" medical plan; <br /> 5. A payment of One Hundred Fifty and no /100 Dollars ($150.00) per month for employees <br /> electing not to enroll in the medical and dental plans OR a payment of One Hundred and <br /> no /100 Dollars ($100.00) per month for electing not to enroll in the medical plan only. <br /> Effective August 1, 1996, the City will contribute the following monthly amounts toward the election <br /> of benefits in the plan: <br /> Employee $1 34.80 per month <br /> Employee Plus One 269.59 per month <br /> Employee Plus Family 358.56 per month <br /> Effective January 1, 1999, the City will contribute the following monthly amounts toward the election <br /> of medical benefits: <br /> Employee $148.28 per month <br /> Employee Plus One 296.55 per month <br /> Employee Plus Family 394.42 per month <br /> Employees electing to enroll in the CalifomiaCare 1 -IMO or Blue Cross Prudent Buyer Plan will be <br /> required to contribute the difference between cost of the premium for the Kaiser Plan and the actual <br /> cost of the medical plan premium. Such employee contribution will be on a pre -tax basis as specified <br /> above. <br /> 3. San Leandro City Employees Association <br /> Effective March 1, 1996, the City will establish an IRS 125 Plan (cafeteria benefit plan) which will <br /> provide the following elements: <br /> 1. Pretax payments of employee contribution toward premium; <br /> 2. Medical Care Flexible Spending Account up to a $2,000 maximum per benefit plan year; <br /> 3. Dependent Care Flexible Spending Account up to a $5,000 maximum per benefit plan year; <br /> 4. A "Core Flex" medical plan; <br /> 5. A payment of One Hundred Fifty and no /100 Dollars ($150.00) per month for employees <br /> electing not to enroll in the medical and dental plans OR a payment of One Hundred and <br /> no /100 Dollars ($100.00) per month for electing not to enroll in the medical plan only. <br /> 24 <br />
