TIMELINES <br /> By October, 1, 2011, Davis Street will have accomplished: <br /> • Provide fifty -two (52) San Leandro clients with crisis intervention and /or short term community <br /> counseling services and access to eligible family resource center services. <br /> • Seventy -five percent (75 %) or forty (40) of the sixty -three (52) San Leandro clients will report a <br /> reduction in the severity or frequency of the presenting issue as supported by a decrease in high -risk <br /> behavior. <br /> By January 1, 2012, Davis Street will have accomplished: <br /> • Provide 104 San Leandro clients with crisis intervention and /or short term community counseling <br /> services and access to eligible family resource center services. <br /> • Seventy -five percent (75 %) or seventy eight (78) of the 104 San Leandro clients will report a reduction <br /> in the severity or frequency of the presenting issue as supported by a decrease in high -risk behavior. <br /> By April 1, 2012, Davis Street will have accomplished: <br /> • Provide 157 San Leandro clients with crisis intervention and/or short term community counseling <br /> services and access to eligible family resource center services. <br /> • Seventy -five percent (75 %) or 118 (118) of the 157 San Leandro clients will report a reduction in the <br /> severity or frequency of the presenting issue as supported by a decrease in high -risk behavior. <br /> By July 1, 2012, Davis Street will have accomplished: <br /> • Provide 211 San Leandro clients with crisis intervention and /or short term community counseling <br /> services and access to eligible family resource center services. <br /> • Seventy -five percent (75 %) or 158 of the 211 San Leandro clients will report a reduction in the severity <br /> or frequency of the presenting issue as supported by a decrease in high -risk behavior. <br /> - 14 - <br /> CDBG Agreement with DSFRC — <br /> Community Counseling Program FY11 -12 <br />
