Measure A Health and Wellness Services 2 September 6, 2011 <br />• Mercy Brown Bag Program — Twice monthly distribution of grocery bags of <br />nutritional food consisting of bread, fresh produce, assorted canned foods, and USDA <br />commodities to low- income seniors. <br />• Health Education Classes such as Diabetes Self Management, Foot Care Clinic, <br />Managing Multiple Medications. These classes are part of the Senior Community <br />Center's ongoing Community Education Series. Topics and speakers vary based on <br />availability of speakers and timeliness of important topics. <br />• Pull Up A Chair Exercise Class — Weekly class includes stretching, balance and light <br />resistance training with bands and weights. <br />2. Fund the Fall Prevention Exercise and Balance Class (Fall Risk Class) offered through <br />Spectrum Community Services. <br />• Each year, more than 1.6 million older Americans go to the emergency room for fall - <br />related injuries. Among older adults, falls are the number one cause of fractures, <br />hospital admissions for trauma, loss of independence and injury - related deaths. <br />• Spectrum Community Services will offer four fall prevention classes per week at the <br />Senior Community Center. Classes include warm -up, cardio training, strength <br />training, balance exercises and cool down. Class also includes education component <br />addressing behavioral (awareness about falls, medication management, and healthy <br />eating habits) and environmental (home safety) factors that can prevent falls. Senior <br />fitness levels are assessed every six months, in addition to self evaluations. The <br />Senior Fitness assessment is a standardized test developed by the Cal State <br />Fullerton's Center for Successful Aging. <br />• The Fall Prevention Class was funded in FY 2010 -11 through a donation. Without <br />the Measure A Grant, this important program would not be offered. <br />Alameda County is contracting for the services listed above and staff will report measurable <br />outcomes as required in the contract.
