Contract No. <br /> EXHIBIT A <br /> DEFINITION OF SERVICES <br /> Contractors shall use funds under this Agreement to provide Health and Wellness Services to San Leandro Seniors. <br /> Contractors shall use funds under this Agreement to fund two part-time staff positions to administer health and <br /> wellness programs and services as well as coordinate fall prevention classes (see Attachment A). <br /> Scope of Work: <br /> Contractor shall provide the following services at the Senior Community Center in San Leandro: <br /> 1. Health Checks <br /> • 30 seniors per month will receive free, drop -in blood pressure and /or other health checks (services <br /> will be coordinated by the Recreational Specialist and /or Leader). <br /> 2. Mercy Brown Bag Program <br /> • Staff shall gather and arrange that 48 low- income seniors per month will each receive a grocery <br /> bag of nutritional food provided through the Mercy Brown Bag Program (the Recreational <br /> Specialist and/or Leader will coordinate efforts with Mercy Brown Bag). <br /> 3. Health Education Classes <br /> • 6 health education classes will be offered and provided per year. <br /> 4. Pull Up A Chair Exercise Class <br /> • 60 seniors per month will attend weekly Pull Up A Chair Exercise Class. Class includes <br /> stretching, balance, light resistance training with bands and weights. <br /> 5. Fall Prevention Exercise & Balance Class <br /> • 300 seniors per month will attend Fall Prevention Courses, offered 4 times /week <br /> Reporting Requirements <br /> Contractor shall provide attendance for classes /services listed under Items 1 — 5 above offered at the Senior <br /> Community Center during the reporting period. In addition, Contractor shall provide a list of all additional health <br /> and wellness programs and services offered at the Senior Community Center during the reporting period. <br /> Contractor shall complete a Fiscal Year Report upon request of the Measure A Oversight Committee. The <br /> Measure A Oversight Committee's role is to annually review Measure A expenditures for each fiscal year and <br /> report to the Board of Supervisors on the conformity of such expenditures to the purposes set forth in the Measure. <br /> The Fiscal Year Report shall provide budget and narrative details in support of the Measure A Oversight <br /> Committee's role. <br /> To raise awareness of the purpose and importance of Measure A funding and to demonstrate transparency, <br /> accountability, and good governance of public funds, every funded organization is encouraged to provide <br /> education about this fact in appropriate and publicly accessible areas in their agency. <br /> County reserves the right to request additional information. <br /> The approval of County to a requested change shall not release Contractor from its obligations under this <br /> Agreement. <br /> • <br /> 1 of 1 <br />
