Page 1 of 21 <br />Alameda CTC Paratransit Program Plan Application <br />Application Period July 1, 2013 through June 30, 2014 <br /> <br /> <br />Annual Paratransit Program Plan Application <br />for Measure B Funding <br />Fiscal Year 2013-2014 (July 1, 2013 through June 30, 2014) <br /> <br />Requirements and Instructions <br />The Alameda County Transportation Commission (Alameda CTC) requires Measure B paratransit <br />fund recipients to participate in an Annual Paratransit Program Plan Review. Recipients are <br />required to complete and submit a program plan application to Alameda CTC that outlines their <br />anticipated expenditures related to delivering paratransit services to seniors and people with <br />disabilities. As part of the Program Plan Review application, recipients may apply for an <br />Implementation Guidelines Assistance (IGA) grant to secure critical funding for the <br />implementation of paratransit services that would otherwise be unavailable without IGA <br />funding. <br />Paratransit Program Plan Application Deadline: March 1, 2013 <br />The Annual Paratransit Program Plan Application includes the following documents: <br />1. Paratransit Program Plan Application (this MS Word document) <br />2. Paratransit Program Plan Attachments including: Tables A, B, C and D (one MS Excel <br />workbook) <br />Submit both files via email by March 1, 2013 to Naomi Armenta: narmenta@alamedactc.org. <br />Hard copies are not required.
