CityofSanLeandroAnnualActionPlan <br />2014 <br />6 <br />OMBControlNo:2506Ͳ0117(exp.07/31/2015) <br /> <br />Provideaconcisesummaryofthejurisdiction’sactivitiestoenhancecoordinationbetween <br />publicandassistedhousingprovidersandprivateandgovernmentalhealth,mentalhealth <br />andserviceagencies(91.215(l)) <br />EveryOneHomeactivitiestoaddressthehousingneedsofthehomeless,andextremelylowͲincome <br />personswithseriousmentalillnessand/orthoselivingwithHIV/AIDScenterontheimplementationof <br />theEveryOneHomePlan.Planningeffortsbeganin2004asauniquecollaborationamongcommunity <br />stakeholders,citiesandAlamedaCountygovernmentagenciesrepresentingthreeseparatecaresystems <br />—homelessservices,HIV/AIDSservices,andmentalhealthservices—thatshareoverlappingclient <br />populationsandarecognitionthatstablehousingisacriticalcornerstonetothehealthandwellͲbeing <br />ofhomelessandatͲriskpeople,andourcommunities.Sinceitspublicationin2006thePlanhasbeen <br />adoptedbytheAlamedaCountyBoardofSupervisorsandallfourteen(14)cities,includingSanLeandro, <br />andendorsedbynumerouscommunityorganizations. <br /> <br />ImplementationoftheEveryOneHomePlanisspearheadedbyacommunityͲbasedorganizationofthe <br />samename.ItisguidedbyaLeadershipBoardcomprisedofjurisdictionalappointedmembersandkey <br />communityconstituenciessuchasconsumers,cities,nonprofitserviceprovidersandhousing <br />developers,businesses,lawenforcement,housingauthorities,andfaithͲbasedorganizations. <br /> <br />EveryOneHomeenvisionsasystemofhousingandservicesinAlamedaCountythat,by2020,ensuresall <br />extremelylowͲincomeresidentshaveasafe,supportiveandpermanentplacetocallhomewithservices <br />availabletohelpthemstayhousedandimprovethequalityoftheirlives.EveryOneHomepartnersare <br />workingonfivecorestrategies: <br /> <br />1)Preventhomelessnessandotherhousingcrises;2)Increasehousingopportunitiesforhomelessand <br />extremelylowͲincomehouseholds;3)Deliverflexibleservicestosupportstabilityandindependence;4) <br />Measuresuccessandreportoutcomes;and5)DeveloplongͲtermleadershipandbuildpoliticalwill. <br /> <br />DescribeconsultationwiththeContinuum(s)ofCarethatservesthejurisdiction'sareain <br />determininghowtoallocateESGfunds,developperformancestandardsforandevaluate <br />outcomesofprojectsandactivitiesassistedbyESGfunds,anddevelopfunding,policiesand <br />proceduresfortheoperationandadministrationofHMIS <br />AlamedaCountyHousingandCommunityDevelopmentDepartmentthroughHMISandleadershipof <br />theEveryOneHomePerformanceManagementCommitteeissupportiveoftheEveryOneHome <br />initiativetoestablishsystemwideoutcomesandtoevaluateeffectivenessofprogramsagainstthose <br />outcomes.Theseoutcomesincludeshorteningtheperiodoftimebeinghomelessandreducingthe <br />recidivismratesforhomelesspeople. <br /> <br />ConsultationwithEveryOneHome,theAlamedaCountywideContinuumofCare,ontheuseof <br />EmergencySolutionsGrant(ESG)funds,beganearlyin2012,whenrepresentativesfromtheCityof <br />Berkeley,theCityofOakland,AlamedaCountyHousingandCommunityDevelopmentDepartment <br />(UrbanCountygrantee),andtheExecutiveDirectorofEveryOneHomeworkedtogethertoimplement <br />thenewESGrequirementsinawaythatwouldbeconsistentcountyͲwideandwouldcontinuea <br />collaborationwhichbeganin2009withtheAmericanRecoveryandReinvestmentAct(ARRA) <br />HomelessnessPreventionandRapidReͲhousing(HPRP)funds.Thiscollaborationresultedinthe <br />creationofthePriorityHomePartnership(PHP),whichwasacountyͲwideprogramtoimplement
