Import all prior loss data dated before July 1, 2014 into a single database. <br /> e, Assist in the preparation of all reports or provide data required by the State of California, <br /> Department of Industrial Relations, Self-Insurance Plans, Public Insurer's Annual Report, the City <br /> of San Leandro's excess Workers Compensation coverage providers, actuarial consultants and <br /> other agents as reasonably required. <br /> f. Provide scheduled reports to be sent electronically to the City of San Leandro each month, each <br /> quarter and upon request. These reports will include, but are not limited to: <br /> • Detailed listing of open claims &closed claims <br /> • Summaries of all open and closed claims <br /> • Listing of first aid only claims <br /> • Claims Cost Detail <br /> • Claims Cost Summary by Year <br /> • Claims by Department <br /> = Claims trending <br /> • Vendor Payment Detail <br /> • <br /> Vendor Payment G...mmary <br /> • Financial reconciliation ledger <br /> • OSHA reports <br /> • Temporary Disability and Disability Payments by Year <br /> • Penalty Reports, including Self-Imposed Penalties <br /> • Ad hoc customized reports that may include, but not be limited to, litigation reports, <br /> settlement reports, loss causation, lost workdays, modified work days. injury types, and/or <br /> use of medical clinics. <br /> g. Quality control program to ensure data integrity and claimant confidentiality. <br /> h.
