(a) Name, address and User classification numbe code (SIC) of applicant; <br />(b) Volume of wastewater to be discharged; <br />(c) Wastewater constituents and characteristics OnGluding but not limited te <br />(d) Time and duration of discharge; <br />(e) Average and maximum wastewater flow rates, including daily, monthly <br />and seasonal variations, if any; <br />(f) Site plans, floor plans, mechanical and plumbing plans and details to show <br />all sewers and appurtenances by size, location and elevation; <br />(g) Description of activities, facilities and plant processes on the premises <br />including all materials, preGesses and types of materials whinh aro that will or could be <br />discharged; <br />(h) Each product produced by type, amount, and rate of production; <br />(i) Number and type of employees, and hours of work; <br />Q) Spill prevention plans; <br />(k) Solvent management plans; <br />(1) Pollution prevention plan; <br />(m) Storm water pollution prevention plan and monitoring plan; <br />(n) Any other information as may be deemed by the City to be necessary to <br />evaluate the permit application. <br />The City will evaluate the data furnished by the User and may require additional <br />information. After evaluation and approval of all the data required, the City may issue a <br />permit, subject to specific terms or conditions provided therein. The holder of all permits <br />issued hereunder shall be subject at all times to all applicable Federal, State, and local <br />laws and regulations. <br />Section 3-14-510. Major Contributing Industries. <br />All major contributing industries proposing to discharge into the City sanitary <br />sewer must obtain a permit within sixty (60) days of the commencement of operation. <br />Section 3-14-515.
