File Number: 15-471 <br />cushions were replaced with preformed rubberized ones per current City standards. <br />In addition, sanitary sewer point repairs were made to sewer mains near four manhole <br />locations along Bancroft Avenue: one between Dutton Avenue and Oakes Boulevard, and the <br />other three at the intersections of Glen Drive, Begier Avenue, and Pala Avenue. These point <br />repairs fixed mains which had settled or had offsets with respect to the sanitary sewer <br />manhole structure. A total of 71 lineal feet of failed 6” and 8” diameter sanitary sewer mains <br />were replaced on this project. <br />Analysis <br />Construction is now complete and was performed in compliance with the contract documents. <br />Previous Actions <br />·On May 6, 2013, by Resolution No. 2013-059, the City Council approved the plans and <br />specifications and called for bids on the subject project. <br />·On May 19, 2014, by Resolution No. 2014-044, the City Council awarded a construction <br />contract to Gallagher and Burk. <br />Applicable General Plan Policies <br />This action is consistent with Streets and Highways Goal No. 16 of the General Plan and <br />Action Item 16.03: Regularly maintain City streets and traffic control devices to ensure that <br />streets operate safely and efficiently. <br />Environmental Review <br />This project is categorically exempt from CEQA per Section 15301(c) - Existing Facilities. A <br />Notice of Exemption was filed with Alameda County on March 3, 2014. <br />Summary of Public Outreach Efforts <br />The contractor distributed flyers to adjacent properties along the project street segments <br />before commencing work. <br />Fiscal Impacts <br />A summary of the purchase order with Gallagher and Burk is listed below: <br />Original Contract: $626,275.00 <br />Change Orders: $94,384.85 <br />Total Contract Amount: $720,659.85 <br />Major change orders involved the following: additional speed cushion installations and a truck <br />detour to avoid trimming existing street trees. <br />The estimated total project cost including design, construction management and pending <br />project closeout is approximately $850,000. <br />Budget Authority <br />The following funding sources were used for this project: <br />1)$190,000.00 - Gas Tax Funds, Account No. 142-38-351, allocated in FY 2013-14 <br />2)$23,056.00 - Gas Tax Funds, Account No. 142-38-357, allocated in FY 2013-14 <br />Page 2 City of San Leandro Printed on 9/2/2015
