<br /> DRAFT City of San Leandro FY 2014‐2015 CAPER 11 <br />OMB Control No: 2506‐0117 (exp. 07/31/2015) <br />CR‐25 ‐ Homeless and Other Special Needs 91.220(d, e); 91.320(d, e); 91.520(c) <br />Evaluate the jurisdiction’s progress in meeting its specific objectives for reducing and ending <br />homelessness through: Reaching out to homeless persons (especially unsheltered persons) and <br />assessing their individual needs <br /> <br />The City uses CDBG funds and City General Funds to provide support services to homeless persons. The City <br />relies and funds the following social services agencies to provide emergency shelter and/or supportive services <br />to homeless persons so that they are able to move into transitional/interim housing, and better yet to <br />permanent housing. <br /> Building Futures with Women and Children <br /> Davis Street Family Resource Center <br /> Family Emergency Shelter Coalition (FESCO) <br /> <br />The City also supports the following social services providers whose services assist City residents from being <br />displaced and becoming homeless. <br /> Neighborhood Solutions <br /> ECHO Housing <br /> <br />Furthermore, the City continues to be a reliable partner with EveryOne Home, the community‐based <br />organization formed to coordinate the implementation of the EveryOne Home Plan. The EveryOne Home Plan <br />is a comprehensive blueprint to end homelessness, including chronic homelessness, by the year 2020. <br /> <br />HOME Consortium jurisdictions expanded their outreach in a number of significant ways over the last year. AC <br />Impact, a direct access to permanent housing from the streets program, launched in August 2013. This <br />partnership between the cities of Fremont, Hayward, Livermore and Oakland and the Continuum of Care with <br />Abode Services as the lead agency, focused on chronically homeless persons whom law enforcement had <br />identified as highly vulnerable and having particularly problematic street behavior. Abode’s outreach team <br />targeted outreach efforts to prioritized individuals and to‐date has permanently housed 27 with 13 vouchers <br />still available. Abode’s mobile outreach team covers all of South and East County and works closely with the <br />human services and law enforcement departments of the cities to identify and contact unsheltered persons. In <br />addition, since the launch of AC Impact, the City of Livermore has invested additional general fund resources to <br />expand street outreach. <br /> <br />The City of Albany also secured the services of the Berkeley Food and Housing Project to conduct outreach in a <br />major encampment in its jurisdiction. <br /> <br />EveryOne Home has worked with providers, Alameda County Housing and Community Development <br />Department and Behavioral Health Care Services to develop a vulnerability screening tool for homeless <br />persons that has identified nearly 200 unsheltered chronically homeless persons for services from housing <br />navigators and outreach teams. <br /> <br />Addressing the emergency shelter and transitional housing needs of homeless persons <br /> <br />Objective: Create Suitable Living Environment <br /> <br />Priority: Maintain and improve the current capacity of the housing and shelter system, expanding <br />transitional and permanent supportive housing, and providing services to homeless individuals and families, <br />including integrated health care system, employment services, and other supportive services.
