- 18 <br />CDBG Consulting Services Agreement <br />for Rebuilding Together Oakland East Bay <br /> Contractors: Providing skilled inspection services, including preparation of work write- <br />ups, scope of work, and project close-out to ensure compliance with local building and <br />safety codes. Monitoring contractors for compliance with local and State health, safety, <br />and building laws. Maintaining a list of State licensed and certified contractors who have <br />a City business license. <br /> <br /> HUD regulations: Providing experience and knowledge of federal HUD regulations and <br />guidelines, specifically compliance with CDBG regulations. <br /> <br /> D. Consultant shall deliver to City within fifteen (15) days after execution of grant documents, <br />copies of homeowners’ Project Summary, Certification Letter, and Itemization Statement. <br /> <br />V. CONSTRUCTION POLICIES AND PERMITS <br /> <br />Consultant shall obtain building permits for structural, plumbing, electrical, heating, or any other <br />repairs that require inspection. Work in excess of $500 shall be performed by licensed contractors. <br />Consultant shall monitor conformance with all state and local laws. <br /> <br /> <br />VI. REPORTING AND MISCELLANEOUS <br /> <br />Consultant shall submit the City-provided Quarterly Reports within fifteen (15) days of the end of <br />each quarter. The quarterly and narrative reports shall include, but not be limited to, the following <br />data elements: <br /> <br />A. Name and address of applicants <br />B. Date of application <br />C. Date of site visit <br />D. Dates of start and completion <br />E. Description of work performed <br />F. Costs of work performed <br />G. Name of contractor <br />H. Income and ethnicity of clients <br />I. Total number of applications <br />J. Total number of units under construction <br />K. Total number of units completed or committed <br />L. Total costs of projects monthly <br /> <br />To ensure timely expenditure of HUD funds, Consultant shall submit monthly reimbursement claims <br />using the City’s invoice or comparable form with documentation of claim including copies of <br />timesheets (denoting specific hours for CDBG work performed, payroll stubs, DE3 or employment or <br />contract letter for personnel costs, invoices or billings. <br /> <br />174
