EXHIBIT A <br /> SCOPE OF SERVICES FOR CITY OF SAN LEANDRO FUNDS <br /> FY 2017-2018(July 1,2017—June 30,2018) <br /> PROJECT GOALS AND OBJECTIVES <br /> Davis Street Family Resource Center will assist the City of San Leandro with various activities related to affordable <br /> housing assistance for homeless, very low-, low-, and moderate-income persons. Goals include assistance with <br /> affordable housing information and helping to provide decent housing and a suitable living environment for eligible <br /> clients. Objectives include assisting clients with affordable housing activities,assisting City staff with dissemination of <br /> information regarding affordable housing units and building/maintaining relationships with property owners/managers. <br /> MAJOR ACTIVITIES <br /> ❖ Maintain formal strategic partnership(s)to target homeless,homebound seniors and very low-income populations <br /> in the City of San Leandro for affordable housing opportunities and activities: <br /> ■ DSFRC will ensure that affordable housing candidates are routed to the family support services program. <br /> ■ DSFRC will coordinate with the Interfaith Homeless Network(IHN)and Alameda County HealthCare Services <br /> Agency School Based Services to provide affordable housing services for the City's homeless. DSFRC will <br /> offer IHN and School Based Services clients services to help stabilize individuals eligible for affordable <br /> housing units. <br /> ❖ Assist in determining eligible tenants for properties with affordable rental units, including the City's below-market <br /> rate(BMR)units: <br /> ■ Educate targeted walk-in clients and clients referred by Alameda County Healthcare Services School Based <br /> Services about available affordable housing subsidies. <br /> ■ Assist applicants with rental applications if necessary. <br /> ■ Maintain updated vacancy list for the City's BMR units— <br /> o Distribute BMR data to Eden I&R regularly <br /> ■ Build and maintain relations with BMR and other owners/property managers and continue participation in the <br /> Alameda County Outreach and Resource Network <br /> ❖ Assist the City with client calls regarding housing information: <br /> ■ Distribute housing information and referral packets, including the City's affordable housing resources, to <br /> clients at the Family Resource Center. <br /> ■ Educate clients about 2-1-1 phone service related to affordable housing. Add information about 2-1-1 on <br /> DSFRC's website and distribute 2-1-1 fliers. <br /> ❖ Attend pertinent City meetings to support affordable housing issues/projects to help educate the public about <br /> affordable housing issues and needs. <br /> PERFORMANCE MEASURES <br /> ❖ Report number of clients assisted with housing search assistance and placed into rental housing units. <br /> ❖ Provide information and referral assistance and informational packets to at least 50 clients needing housing <br /> assistance. <br /> ❖ DSFRC staff will connect with Alameda County Healthcare Services Agency School Based Initiative to help provide <br /> fair housing information to clients seeking housing assistance. <br /> ❖ Submit semi-annual housing/homeless reports regarding activities related to housing assistance for clients. 1St <br /> semi-annual report due on or before January 15, 2018.
