Consulting Services Agreement between City of San Leandro and 12/18/2017 <br />ConvergeOne, Inc. for Polycom Videoconferencing Equipment Exhibit A – Page 7 of 10 <br />Knowledge Transfer <br />Standard installation and documentation includes all manufactures' documentation as shipped with product <br />and a onetime 30 minute overview of equipment/room functionality by the CONVERGEONE, INC. <br />installation team and client contact while the CONVERGEONE, INC. installation team is on-site for the <br />implementation of the project upon system completion (same day required). <br />Change Control Procedure <br />Despite good project planning, design and review, project plans often require some degree of change at <br />some point. These changes are handled using Change Order requests, which must be agreed upon by all <br />parties to the contract before such work can be performed. <br />Either CONVERGEONE, INC. or the client may initiate a Change Order for any deliverable, work <br />requirement, assumption or dependency that is part of the project. All requests must be in writing and <br />handled by the CONVERGEONE, INC. Project Manager. CONVERGEONE, INC. will review the change <br />and provide pricing as applicable before proceeding. The CONVERGEONE, INC. Project Manager may <br />also engage project team members to assess the impact of the change. Agreed changes must be approved <br />in writing by an authorized representative of the client, via email or a modified purchase order. <br />CONVERGEONE, INC. Standard Service Hours <br />Unless indicated within this SOW, pricing is based on the assumption that Services will be performed <br />between 8:00 AM and 5:00 PM local site time, Monday-Friday, excluding CONVERGEONE, INC.- <br />designated holidays (“Standard Service Hours”). Work performed outside Standard Service Hours may be <br />subject to overtime charges at the following rates: <br />Overtime (1.5 times)Premium Overtime (2.0 times) <br />Evenings, Monday-Friday 5:00 PM-8:00 AM Saturday 12:00 AM (midnight)–Monday 8:00 AM <br />Saturday, 8:00 AM–12:00 AM (midnight)CONVERGEONE, INC.-designated holidays <br />148
