EXHIBIT D <br /> SCOPE OF WORK <br /> 1. All activities funded with CDBG funds must meet one of the CDBG program's National Objectives: benefit <br /> low- and moderate-income persons; aid in the prevention or elimination of slums or blight; or meet <br /> community development needs having a particular urgency,as defined in 24 CFR 570.208. <br /> 2. The CDBG funds approved under this Agreement shall only be used in assisting Housing Consortium of <br /> the East Bay in rehabilitating the Property. <br /> 3. The Participant shall follow all provisions and keep all records set forth in Exhibit F and Exhibit G of this <br /> Agreement. <br /> 4. The Participant shall request any changes to the Scope of Work, Time Schedule or Budget in writing. The <br /> City Manager or his/her designee may approve requests at his/her discretion. <br /> 5. List of Work for which Participant may use CDBG funds: <br /> Participant shall use the CDBG funds subject to this contract to fund rehabilitation of the Property, <br /> commonly known as 342 West Joaquin Avenue, San Leandro, CA 94577 including the following <br /> improvements: installation of automatic doors and related parts to meet American with Disabilities <br /> Act(ADA)requirements. Participant shall have primary responsibility for carrying out the <br /> rehabilitation or improvements, including, but not limited to, securing additional financing and <br /> obtaining planning approvals. <br /> 6. Program Monitoring and Reporting <br /> The Participant will perform periodic and final site visits to ascertain that the approved Scope of Work is <br /> proceeding properly and satisfactorily. City staff will monitor the performance of the Participant against goals <br /> and performance standards stated in the Scope of Work. Substandard performance as determined by the <br /> City will constitute noncompliance with this Agreement. Contract suspension or termination procedures will <br /> be initiated if action to correct such substandard performance is not taken by the Participant within a <br /> reasonable period of time after notification by the City. <br /> Participant shall submit to the City written quarterly and narrative reports which shall be due on the fifteenth <br /> (15th) day of the month immediately following the report quarter. The quarterly and narrative reports shall <br /> include, but not be limited to,the following data elements: <br /> A. Title of program or project and description of goals and activities. <br /> B. Service areas(i.e.,citywide,applicable census tracts,etc.). <br /> C. Beneficiaries' Data: See attached Quarterly Performance and Narrative Reports, Exhibit E. <br /> D.
