City of San Leandro <br />Meeting Date: June 4, 2018 <br />Resolution - Council <br />Agenda Section:File Number:18-270 CONSENT CALENDAR <br />Agenda Number: <br />TO:City Council <br />FROM:Jeff Kay <br />Interim City Manager <br />BY:Jeff Kay <br />Interim City Manager <br />FINANCE REVIEW:Not Applicable <br />TITLE:RESOLUTION Authorizing the City Manager to Execute a Third Amendment to <br />the Emergency Medical Services First Responder Advanced Life Support <br />Services Agreement with the County of Alameda <br />WHEREAS, on November 1, 2011, the City of San Leandro (the “City”) and the County of <br />Alameda (the “County”) executed an Emergency Medical Services First Responder Advanced <br />Life Support Services Agreement (the “Agreement”) for the City to provide, and receive funding <br />for, First Responder Advanced Life Support (“FRALS”) Units and to serve as a designated <br />provider of FRALS Services; and <br />WHEREAS, on or around May 24, 2016, the City and County executed the First <br />Amendment to the Agreement (the “First Amendment”) in order to extend the term of the <br />Agreement and revise the revenue and support provided to the City for the services provided <br />under the Agreement; and <br />WHEREAS, on or around November 1, 2017, the City and County executed the Second <br />Amendment to the Agreement (the “Second Amendment”) in order to extend the term of the <br />Agreement, revise the revenue and support provided to the City for the services provided under <br />the Agreement, and transfer ownership of Alameda County Emergency Medical Services Agency <br />equipment to the City at no cost; and <br />WHEREAS, the City and the County desire to revise the Agreement to extend the term of <br />the Agreement to June 30, 2019 in order to permit continuance of FRALS Services from the City <br />and to confirm that payments from the County for the services provided will no longer be provided; <br />and <br />WHEREAS, the City and the County desire to execute an additional amendment to the <br />Agreement to reflect the proposed revisions. <br />NOW, THEREFORE, the City Council of the City of San Leandro does RESOLVE as <br />Page 1 City of San Leandro Printed on 5/30/2018 <br />195
