<br />C-1 <br />APPENDIX C <br /> <br />$_________ <br />San Leandro Public Financing Authority <br />2018 Lease Revenue Bonds <br /> <br /> <br />WRITTEN REQUISITION FROM PROJECT FUND <br /> <br />REQUISITION NO. _____ <br /> <br /> <br /> <br />The undersigned hereby states and certifies that: <br /> <br />(i) I am an “Authorized Representative,” as such term is defined in <br />that certain Indenture of Trust, dated as of December 1, 2016, by and between <br />the Authority and the Trustee, as supplemented and amended by the First <br />Supplemental Indenture of Trust, dated as of December 1, 2018, by and <br />between the Authority and the Trustee (as so supplemented and amended, the <br />“Indenture”), and as such, am familiar with the facts herein certified and am <br />authorized to certify the same; <br /> <br />(ii) under Section 13.06 of the Indenture, the undersigned hereby <br />requests and authorizes the Trustee to disburse from the Project Fund <br />established under the Indenture to each payee designated on Schedule A <br />attached hereto and by this reference incorporated herein, the amount set forth <br />opposite such payee, for payment or reimbursement of previous payment of a <br />Project Cost, as described on attached Schedule A; and <br /> <br />(iii) no portion of the amount herein requested to be disbursed was <br />set forth in any Written Requisition previously filed requesting disbursement for <br />Project Costs; <br /> <br />(iv) capitalized terms used but not defined herein are defined in the <br />Indenture. <br /> <br /> <br />Dated: ______, 20___ <br /> <br />CITY OF SAN LEANDRO <br /> <br /> <br />__________________________________B <br />y: ________________________ <br />Its: Authorized Representative
