The CONTRACTOR will be responsible for the design and production of complaint <br />investigation report forms (final design is subject to CITY approval), for the maintenance of <br />a complaint log and the submission of a monthly complaint status summary report. <br />Q. DATA COLLECTION AND REPORTING <br />The CONTRACTOR will collect and maintain all operating and performance data required <br />by the CITY. All reports and/or background data will be certified by the CONTRACTOR <br />to be accurate. <br />The CONTRACTOR shall collect and maintain completed daily driver manifests and <br />dispatch logs. These will provide the base data for the monthly and annual performance and <br />operating reports, fare box reconciliation, and monthly CONTRACTOR billings. <br />The CONTRACTOR will maintain a set of completed, readable driver manifests and <br />dispatch logs for a period of three (3) years, and upon request be made available to the CITY <br />for planning and auditing purposes. <br />CONTRACTOR shall submit monthly reports to the CITY. CONTRACTOR shall have the <br />ability to transmit statistical reports via e-mail in a format that allows CITY to manipulate <br />data as needed for reporting and operational purposes. CONTRACTOR will be asked to <br />collect and report data in format as required by ACTC and may be asked to collect further <br />data as required by ACTC. Such reporting may include financial records related to the <br />delivery of services, and operating service statistics. The CONTRACTOR must be prepared <br />to provide operating statistics that include, but are not limited to, the following applicable <br />data sought by ACTC: <br />1. Individual trips provided to ambulatory passengers <br />2. Individual trips provided to non-ambulatory passengers <br />3. Other specified trips provided. <br />4. Attendants riding along on trips provided. <br />5. Companions riding along on trips provided. <br />6. Trips provided on behalf of other paratransit programs by the CITY. <br />7. Vehicle revenue hours for trips provided under this AGREEMENT. <br />8. Vehicle miles for trips provided under this AGREEMENT. <br />9. Number and percent of contractor No -Shows. <br />10. Percent of early, late and on-time scheduled stops. <br />11. Percent trips over the "ride -time" standard <br />12. Average ride -time <br />13. Number of vehicle/property damage occurrences only <br />14. Number of personal injury occurrences only <br />15. Number of vehicle/property damage and personal injury occurrences. <br />16. Total accidents and incidents. <br />17. Vehicle miles per accident/incident <br />18.
