Annual Action Plan <br />2019 <br />21 <br />OMB Control No: 2506-0117 (exp. 06/30/2018) <br />Description The San Leandro Shelter is a 30-bed homeless shelter. While housed at <br />San Leandro Shelter, residents receive a range of services including <br />meals, on-site case management, housing search assistance, individual <br />therapy, life skills coaching, domestic violence and parenting support <br />groups, evening childrenâ¿¿s activities and access to health care <br />services. <br />Target Date 6/30/20202021 <br />Estimate the number <br />and type of families <br />that will benefit from <br />the proposed activities <br />100 women and children clients annually. <br />Location Description San Leandro Shelter, 501 Davis Street, San Leandro CA <br />Planned Activities The capital improvement project will improve the program’s <br />accessibility for residents with mobility impairments, to enhance safety <br />of the facility, to protect and extend the life of the structure and to <br />increase the quality of life of the clients of the San Leandro Shelter. This <br />will be accomplished by replacing the exterior ramp, stairs and entryway <br />landing; rebuilding four bathroom shower stalls and subfloors, including <br />ADA shower stall; and replacing the floors in the common area. <br />12 Project Name Rental Assistance Program <br />Target Area City-Wide <br />Goals Supported Community Development Needs Goals-Providing Enhanced Public <br />Services <br />Needs Addressed Community Development Needs - Public Services <br />Funding $451,972 <br />Description The City of San Leandro will establish a Rental Assistance <br />Program benefiting low- and moderate- income households <br />(CDBG definition <80% of the Area Median Income) who face <br />eviction or financial hardship as a result of COVID-19. The goal of <br />this program is to maintain housing stability of San Leandro <br />residents. Current HUD guidance allows for three months of rental <br />housing assistance of CDBG eligible recipient households. <br />Target Date 6/30/2022 <br />78
