The report <br />identifies physical barriers to accessibility in City-owned <br />facilities and how the City may remove those barriers to <br />facilitate the opportunity of access to all individuals. <br />Table 3– Other local / regional / federal planning efforts <br /> <br />Describe cooperation and coordination with other public entities, including the State and any <br />adjacent units of general local government, in the implementation of the Consolidated Plan <br />(91.215(l)) <br />The Alameda County HOME Consortium, which consists of the Alameda County Housing and Community <br />Development Department (HCD) as the lead agency and the following cities of Alameda, Fremont, <br />Hayward, Livermore, Pleasanton, San Leandro, and Union City, and the Urban County, which includes <br />the cities of Albany, Dublin, Emeryville, Newark, and Piedmont, and the unincorporated areas of the <br />County, met to coordinate planning efforts for the adoption of the Consolidated Planning cycle starting <br />July 1, 2020 through June 30, 2025 and the Annual Action Plan for FY 2020-2021. <br />Narrative <br />252
