Provide 5 San Leandrans with a San Leandro based community support group for victims in an <br />accessible location and/or Building Futures With Women and Children 6-hour curriculum for those <br />seeking to remove restraining orders; <br />3. Provide 5 presentations (to approximately 20 or more people, depending on the Public Health Order <br />and remaining in compliance with practicing social distances. Alternately, presentations and webinar <br />will be made available remotely using distance learning tools appropriate for maintaining <br />confidentiality and privacy.) to San Leandro businesses, service providers, workplaces, and schools <br />on the dynamics of intimate partner violence and safe ways to help; <br />4. Provide 1,500 community members with outreach communication including a postcard in three <br />languages in October and February and also via social media. <br />5. Provide 3-5 families with emergency hotel stays includes food, clothing and transportation. Support is <br />provided. <br /> <br />TIMELINES <br /> <br />• By September 30, 2020, develop and print postcards and fliers; provide 2 presentations; start <br />support groups, serve 6 new residents. Provide 3 presentations, serve 6 new residents. <br /> <br />• By December 31, 2020 begin weekly domestic violence support group for self-identified men who <br />report being victims of intimate or family violence, provide 2 healthy relationships presentations to <br />marginalized and hard-to-reach community members and serve 6 individuals or family residents of <br />San Leandro. <br /> <br />• By March 30, 2021 provide materials for Teen Dating Violence Awareness Month, Serve 6 new <br />residents. <br /> <br />• By June 30, 2021 print and distribute fliers, provide 3 presentations, serve 4 new residents. <br /> <br />REPORTS AND MISCELLANEOUS <br /> <br />1. Agency must submit the City-provided Quarterly Reports within 15 days of the end of <br />the each quarter. The Quarterly Reports shall reflect the number of persons served, and <br />shall also include a narrative section. The reports shall be submitted even if there are no <br />specific numbers to report, and the narrative report should describe why no specific <br />numbers are reported. <br /> <br />2. To ensure timely expenditure of HUD funds, Agency must submit monthly <br />reimbursement claims using the City's invoice or comparable form with documentation of <br />claim including copies of time sheets (denoting specific hours for CDBG work performed), <br />payroll stubs, DE3 or employment or Agreement letter for personnel costs, invoices or <br />billings. <br /> <br />3. Housing Services Division staff will conduct CDBG desk monitoring on an ongoing <br />basis based upon the City’s CDBG monitoring plan. <br /> <br />4. Human Services staff and members of the Human Services Commission (HSC) and <br />Housing Services Division staff will conduct at least one monitoring site visit per year. <br />151
