EXHIBIT A <br />SCOPE OF WORK <br />Term of Contract: July 1, 2020 — June 30, 2021 <br />Agency: FAMILY VIOLENCE LAW CENTER <br />Address: 47027 1h Street <br />Oakland, California 94612 <br />Contact Person: Erin Scott, Executive Director <br />Phone: 510-208-0220 <br />Project Name: Family Violence Prevention Program <br />PROJECT GOALS AND OBJECTIVES <br />FVLC provides San Leandro residents fleeing family violence with free legal services, and in-kind <br />supportive non -legal services. <br />MAJOR ACTIVITIES <br />FVLC's services include providing legal information, advice and consultation; paperwork <br />preparation; and legal representation in restraining order and related family law matters (for <br />example, incorporated child custody and family support orders, and move -out orders requiring the <br />batterer to move out and stay away from the family residence, among others). <br />PERFORMANCE MEASURES <br />SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT: <br />1. Direct legal assistance to at least 40 San Leandro families experience domestic violence. These <br />services protect survivors from the loss of residence, the return of the batterer, and family <br />disintegration. At least 8 domestic violence survivors will receive assistance obtaining court orders. <br />2. Housing services (such as obtaining move -out orders as part of restraining orders, obtaining <br />funds to relocate, and emergency housing and relocation assistance) to at least 10 San Leandro <br />residents experiencing domestic violence. These services enable survivors to permanently leave <br />abusive relationships without becoming homeless. <br />3. In-kind crisis counseling, safety planning, assistance with Victims of Crime applications and/or <br />services referrals to at least 16 San Leandro survivors of domestic violence. These services, <br />funded by multiple other sources, help survivors remain safe and connect them with FVLC's longer <br />term legal, counseling and case management services. <br />TIMELINES <br />
