EXHIBIT A
SCOPE OF SERVICES
*Modifiedfrom original version to includefundraising activities for additional senior outreach,
as noted in summary question 2b of our original narrative. We will do our best to raise
additional funds for up to 50 additional seniors however if funds are not acquired, our service
level will remain at 381 San Leandro seniors.
a. Project Goals and Objectives (Describe the project or program and how participants will
benefit.):
Project Goal #1: Increase food security for the 17% of San Leandro's elders who live below
150% of the poverty level
Service gap met in the Human Services Gap Analysis: Elders who live in poverty are more likely
to be food insecure. This can lead to a range of problems including health issues (depression,
asthma, congestive heart failure and more) which seniors are more susceptible to. The subsequent
medical costs associated with illness lead to increased economic vulnerability.
Objective 1.1: During each year of the grant period, MBBP will remain steadfast in conducting 24
grocery distributions serving 381 low-income San Leandro seniors
Objective 1.2: Throughout the grant period, MBBP will conduct Calfresh outreach with all eligible
semors
Objective 1.3: Over the course of the grant period, MBBP will conduct fundraising activities that
support 3 outreach activities per year to recruit up to 50 new seniors.
Project Goal #2: Increase the accessibility of services for San Leandro's Srutnish and Asian
language speaking senior population
Service gap met in the Human Services Gap Analysis: 59% of Spanish speaking San Leandro
seniors and 79% of Asian and Pacific Island language speaking seniors speak English 'less than
very well'. Thus, it is important that this population are provided services in languages other than
English.
Objective 2.1: Over the course of the grant period, MBBP will conduct fundraising activities that
support 3 outreach activities per year targeting Spanish speaking seniors (no outreach will be
conducted with Asian language speaking seniors as they comprise the largest percentage of current
recipients)
Objective 2. 2: During each year of the grant period, MBBP will ensure program accessibility for
30 Spanish speaking seniors and 150 Asian language speaking seniors
Project Goal #3: Decrease the food expense of seniors who spend over 30% of their income
on rent, allowing them to save their money for rent, medical bills and other essentials
Service gap met in the Human Services Gap Analysis: Nearly one in three San Leandro older
adults is a renter, and of those 57% were cost-burdened in 2010-14. This population is therefore
more likely to experience economic vulnerability and subsequent food insecurity.
Consulting Services Agreement between City of San Leandro and
MERCY RETIREMENT AND CARE CENTER for CAP Grant
Last revised 09/12/2019
Exhibit A -Page 1 of 4
