SCOPE OF SERVICES CONTINUED <br />Objective 3.1: Over the course of the grant period, MBBP will conduct fundraising activities that <br />support 3 outreach activities per year for seniors who rent <br />Objective 3.2: During each year of the grant period, MBBP will serve 200 seniors who rent <br />b. Describe the Activities and Services provided through this project or program. <br />Primary Activity and Service: MBBP senior volunteers and staff conduct twice a month <br />grocery distribution for low-income San Leandro seniors. <br />Pre-distribution Activities: <br />)o> Call-em-All makes reminder calls for interested senior recipients with the date of their <br />pickup <br />)o> MBBP conducts fundraising activities which support ongoing outreach, as described in <br />section 5 be low <br />)o> Senior volunteers translate outreach materials into Spanish and Asian languages <br />Distribution Activities and Services: <br />)o> MBBP staff pickup groceries from Alameda County Community Food Bank (ACCFB) <br />and drive to distribution sites <br />)o> Senior volunteers and MBBP staff unload truck, display or bag groceries as appropriate , <br />and oversee distribution <br />)o> Senior volunteers register and/or check-in all senior recipients using Link2Feed <br />)o> Senior volunteers provide translation services for Spanish and Asian language speakers <br />)o> Senior recipients at public sites select groceries they would like <br />)o> MBBP staff and senior volunteers deliver to the low-income housing complexes as <br />needed <br />)o> MBBP partners with ACCFB to conduct Calfresh outreach and distributes farmer 's <br />markets vouchers as available (given once per year) <br />)o> Senior recipients fill out annual survey <br />Post-distribution Activities: <br />)o> MBBP staff analyzes Link2Feed data, tracking senior attendance by the relevant criteria <br />(languages spoken, housing status, new or continued recipient, etc.) and reports data as <br />appropriate <br />)o> MBBP submits quarterly or biannual CAP reports in format provided by the City of San <br />Leandro <br />c. Performance Measures Describe how your proposed project will improve the San Leandro <br />community. Provide specific data projections. For example: the number of meals provided or <br />participants served.
