EXHIBIT A-5 <br />CITY INSTALLATION GUIDELINES <br />[To be updated with each Pole License.] <br />Licensee shall install a dedicated conduit for its use; however, Licensee may, in lieu of <br />installing a dedicated conduit, use the City's existing non -traffic signal conduits, subject to the <br />allowable conduit fill percentage under the NESC and the review and approval of the City's <br />Public Works Department which may deny Licensee's use based on (i) the City's reasonable <br />safety concerns; or (2) the age of City conduit and probability of Licensee's use causing <br />damage to such conduit and/or cable; or (3) conduit capacity needed for pending City <br />projects.. Licensee is not permitted to install or arrange for installment of external conduits on <br />any City Poles. <br />Licensee may share the City's power conductor from PG&E (or other servicing <br />electricity utility) point of feed to the City Pole. Licensee will then splice and fuse either at the <br />City Pole electrical box or at the hand hole at the base of the City Pole and run wires to the <br />Licensee's meter located on the City Pole. Licensee will also run fiber through the electrical <br />box, conduit and pole to the Licensee's Equipment. <br />Licensee shall be responsible for repairing any City conduits that Licensee or its <br />Agents damage during installation of electrical facilities, including pulling of wires into street <br />light conduits. If following Licensee's failure to make such repairs within thirty (30) days of <br />notice from City, the City's Public Works Department makes repairs to remedy damage <br />caused by Licensee or its Agents, the City will charge Licensee the full cost of those repairs <br />by notice with evidence of the City's costs. <br />The City will inspect Licensee's service installations to ensure compliance with <br />Approved Plans, Specifications, and the NESC. Licensee agrees to make any repairs or <br />modifications to its service installations that are necessary to ensure compliance with the <br />Approved Plans and Specifications. <br />Licensee shall provide the City's Engineering and Transportation Department with as - <br />built drawings showing all circuits installed by Licensee in existing street lights or conduits <br />promptly after installation is complete. Licensee shall provide a laminated copy of the as -built <br />drawings to the City's Public Works Department. <br />Licensee shall provide the City's Engineering and Transportation Department with the <br />final coordinates and/or digital GIS shape file for inclusion in the City's GIS inventory. <br />Licensee shall not open any City pull boxes unless a member of the City's Public Works <br />Department is present or City's Engineering and Transportation Department representative <br />approves opening the pull boxes unattended by such maintenance staff.
