<br />Non-Professional Services Agreement between March 1, 2021 <br />City of San Leandro and Advanced IPM —Exhibit A Page 2 of 2 <br />Authorized Work Days and Hours <br />All work shall be done Monday through Friday between the hours of 7:00 a.m. to 4:00 p.m. excluding <br />designated City holidays and observances, unless otherwise directed by the Building Supervisor. <br /> <br />The following are the designated City holidays and observances: <br /> <br />New Year’s Day January 1 <br />Martin Luther King’s Birthday Third Monday in January (observed) <br />Lincoln’s Birthday Second Monday in February (observed) <br />Washington’s Birthday Third Monday in February (observed) <br />Memorial Day Last Monday in May <br />Independence Day July 4 <br />Labor Day First Monday in September <br />Veterans’ Day November 11 <br />Thanksgiving Fourth Thursday and Friday in November <br />Holiday Closure Christmas Eve through New Year’s Day <br /> <br />Service Inspection Reports <br />Contractor shall provide to the Deputy Public Works Director a monthly service report for all City Facilities by <br />the fifth day of the succeeding month. The report shall include: <br /> <br />1. Date and location of service; <br />2. Technician name and license number; <br />3. Service description including indoor and/or outdoor application ; <br />4. List of pesticides. Include the following information: EPA #, active ingredient, undiluted quantity, and <br />target pest; and <br />5. Notation if no pesticides were applied <br /> <br />The report shall be provided in an electronic format. <br /> <br /> <br /> <br /> <br /> <br /> <br />103
