Amendment No. 3 to Consulting Services Agreement between City of San Leandro and TJKM Transportation Consultants for East 14th Street/144th Avenue and East 14th Street/Joaquin Avenue Signal Project Page 1 of 3 AMENDMENT NO. 3 TO CONSULTING SERVICES AGREEMENT BETWEEN THE CITY OF SAN LEANDRO AND TJKM TRANSPORTATION CONSULTANTS FOR EAST 14TH STREET/144TH AVENUE AND EAST 14TH STREET/JOAQUIN AVENUE SIGNAL PROJECT PROJECT NO. 2019.5615 AND 2018.5690 This Amendment No. 3 (“Amendment”) is made by and between the City of San Leandro (“City”) and TJKM Transportation Consultants (“Consultant”) (together sometimes referred to as the “Parties”) as of ________________________, 2021, and amends that certain Consulting Services Agreement (“Agreement”) dated February 20, 2019, between the Parties. WHEREAS, City and Consultant executed the Agreement, pursuant to which Consultant provided City certain consulting services with regard to the East 14th Street/144th Avenue and East 14th Street/Joaquin Avenue Signal Project, and WHEREAS, the Parties want to amend the Agreement to add Task 3.6 to Pothole at up to five (5) locations; and NOW THEREFORE, for good and valuable consideration, the sufficiency of which is hereby acknowledged, the Parties hereby agree to amend the Agreement as follows: 1. Section 2 of the Agreement entitled “Compensation” is hereby amended to pay Consultant a sum not to exceed $74,674 (an increase of $6,119); and 2. Exhibit A of the Agreement entitled “Scope of Services” is hereby amended as follows: a) Task 3.6 for East 14th Street / Joaquin is added thus: Consultant shall perform Utility Potholing at up to five (5) locations. 3.
