EXHIBIT A <br /> <br />SCOPE OF WORK <br /> <br />Term of Contract: July 1, 2021 – June 20, 2022 <br /> <br />Agency: Davis Street Family Resource Center <br /> <br />Address: 3081 Teagarden Street <br /> San Leandro, CA 94577 <br /> <br />Contact Person: Rose Padilla-Johnson, CEO <br /> <br />Phone: (510) 347-4620 x100 <br /> <br />Project Name: Family Support Services – Basic Needs Services <br /> <br /> <br />PROJECT GOALS AND OBJECTIVES <br /> <br />The Family Support Services Program will provide “Basic Needs” services to San Leandro <br />community members. This includes emergency food and clothing and other services. The provision <br />of emergency services to low-income people in San Leandro enables these individuals and <br />households to move out of poverty and into self-sufficiency. <br />The DSFRC Community Counseling Program is an outpatient mental health clinic providing the full <br />spectrum of mental health services across the lifespan. This program specializes in providing <br />culturally relevant intervention (individual, couples, family, and group modalities) and prevention <br />services to enhance the overall quality of life for clients. Crisis, short-term, and long-term <br />therapeutic services are provided. Our therapeutic services are designed to work collaboratively <br />with our clients to identify barriers and strategies to enhance their capacity to live full lives. <br /> <br />MAJOR ACTIVITIES <br /> <br />Households will receive five days’ worth of groceries up to twice a month (boxes of groceries <br />include: canned goods, produce, bread, rice and pasta, dairy, and protein items), and emergency <br />clothing and household items. Households also have access to other services provided within <br />DSFRC (housing, childcare, counseling, and medical/dental services, etc.), and they are also <br />provided with linkages to other services among DSFRC local, county, and state resources and <br />partnerships. <br />DSFRC Community Counseling Program will provide culturally relevant crisis intervention and short- <br />term counseling to individuals, households, and couples across the lifespan. All clients will be <br />provided access to all services within the family resource center-basic needs, medical, and child care <br />services. <br /> <br />PERFORMANCE MEASURES <br /> <br />SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT: use evaluation tools that consist <br />of a client intake interview entry and exit survey, follow-up with subsequent visit(s) and re- <br />certification, and documentation of frequency of visits. To track this grant’s outcomes, sub-recipient <br />will monitor client’s housing and employment status.
