<br />Consulting Services Agreement between City and Consultant Last revised 06/28/2019 <br /> Exhibit A – Page 1 of 1 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />GENERAL SERVICES <br /> <br /> Consultant shall provide trained and experienced forensic scientists and technicians to perform <br />toxicology services, and furnish all supplies and equipment necessary to accomplish services <br />as described in this exhibit. <br /> Consultant shall notify the requesting San Leandro Police Department investigator within one <br />(1) business day of the conclusion of a service request, and respond to messages (telephone, <br />fax or e-mail) left by the City or its agents within two (2) business days of its receipt. <br /> Consultant shall provide the City with the necessary specimen containers, evidence labels, and <br />appropriate shipping containers for the transport of specimens from the City to Consultant. <br /> Consultant shall offer drug testing on blood, and if necessary, urine samples as specified for <br />DUI and Drug related arrests. <br /> Consultant will arrange and pay for weekly pickups through an independent courier service. If a <br />holiday falls upon the normally scheduled pickup day, samples will be picked -up the next <br />business day. <br /> Consultant will be responsible for chain of possession of evidence while in its care only. Any <br />samples invalidated in court because of chain of possession errors while in City's care will <br />become the burden of City. <br /> All forensic biological specimens will be stored for a period of two years unless instructed <br />otherwise by City on an individual case basis. <br /> Consultant will dispose specimens after two years in accordance with all applicable <br />government regulations. <br /> All Consultant toxicology reports must be accurate and easy to read, using common units of <br />measure. Consultant will include reliable drug ranges for reference purposes. The City <br />understands that human error is possible, however, and any and all reporting errors must be <br />revised/amended immediately following discovery. <br /> Consultant turn-around-time (TAT) is generally 5 to 10 calendar days from the time that the <br />sample is received in the laboratory. Phone inquiries concerning the status of a particular <br />sample are welcomed. <br /> A report is issued and mailed as soon as testing is completed. City may receive reports via <br />facsimile or e-mail to improve TAT. <br /> Consultant agrees that under any circumstance, it will not disclose any case information to any <br />person who is not authorized to receive case information by the City. <br /> <br /> <br /> <br /> <br />DocuSign Envelope ID: FBCF2283-E6FF-48BD-A530-57BCB6D6C824
