<br />Amendment No. 1 to Consulting Services Agreement Page 1 of 3 <br />between City of San Leandro and DSFRC Homeless Task Force <br />AMENDMENT NO. 1 TO CONSULTING SERVICES AGREEMENT BETWEEN <br />THE CITY OF SAN LEANDRO AND <br />DAVIS STREET FAMILY RESOURCE CENTER (DSFRC) <br />FOR <br />HOMELESS TASK FORCE <br /> <br /> <br />This Amendment No.1 is made by and between the City of San Leandro (“City”) and Davis Street <br />Family Resource Center (“Consultant”) (together sometimes referred to as the “Parties”) as of June 1, <br />2021, and amends that Consulting Services Agreement (“Agreement”) dated July 1, 2020, between the <br />Parties. <br /> <br />WHEREAS, City and Consultant have executed the Agreement, pursuant to which Consultant has <br />provided certain consulting services to City with regard to the Homeless Task Force, and <br /> <br />WHEREAS, the Parties desire to amend the Agreement to increase rental assistance services. <br /> <br />NOW THEREFORE, for good and valuable consideration, the sufficiency of which is hereby <br />acknowledged, the Parties amend the Agreement as follows: <br /> <br />1. Section 2 of the Agreement entitled “Compensation” is hereby amended to pay Consultant a <br />sum not to exceed $48,469.08 ($18,469.08 for rental assistance and $30,000 for basic needs); and <br /> <br />2. Exhibit A of the Agreement entitled “Scope of Services” is hereby amended to read: <br /> <br />Consultant shall provide Basic Need services to the San Leandro community, by providing <br />emergency services to assist those who are at risk of or who are already experiencing <br />homelessness in order for them to reintegrate in the community. <br /> <br />Major Activities <br />• Provide transportation assistance such as emergency bus tickets, emergency ride assistance <br />(Uber or Lyft), bus passes, emergency gas cards and or clipper cards up to 25 per month <br />• Payment assistance for past due rent with a 3 day notice to evict for up to 3 clie nts per year <br />• Offer 1-3 day motel vouchers for up to 15 clients per year <br />• Unforeseen circumstance situation assistance such as car impoundment (if being used as <br />primary residence), storage lock out, or major car repair for up to 3 clients per year. <br />• One time utility assistance program for up to 5 clients per year <br />• Box lunch station once a week that will provide a pre packed ready to eat meal for up to 15 <br />clients <br />• One time deposit assistance for up to 2 clients yearly (not to exceed $4,000 per year) <br />• Subsidizing rent to low income individuals/families residing within the boundaries of th e City of <br />San Leandro, up to $18,469.08 a year. Clients do not need to be clients of DSFRC to apply. Clients <br />will be assessed and assisted based on need, not by size of their rental unit.
