Amendment No. 1 to Consulting Services Agreement Page 1 of 3 between City of San Leandro and DSFRC Homeless Task Force AMENDMENT NO. 1 TO CONSULTING SERVICES AGREEMENT BETWEEN THE CITY OF SAN LEANDRO AND DAVIS STREET FAMILY RESOURCE CENTER (DSFRC) FOR HOMELESS TASK FORCE This Amendment No.1 is made by and between the City of San Leandro (“City”) and Davis Street Family Resource Center (“Consultant”) (together sometimes referred to as the “Parties”) as of June 1, 2021, and amends that Consulting Services Agreement (“Agreement”) dated July 1, 2020, between the Parties. WHEREAS, City and Consultant have executed the Agreement, pursuant to which Consultant has provided certain consulting services to City with regard to the Homeless Task Force, and WHEREAS, the Parties desire to amend the Agreement to increase rental assistance services. NOW THEREFORE, for good and valuable consideration, the sufficiency of which is hereby acknowledged, the Parties amend the Agreement as follows: 1. Section 2 of the Agreement entitled “Compensation” is hereby amended to pay Consultant a sum not to exceed $48,469.08 ($18,469.08 for rental assistance and $30,000 for basic needs); and 2. Exhibit A of the Agreement entitled “Scope of Services” is hereby amended to read: Consultant shall provide Basic Need services to the San Leandro community, by providing emergency services to assist those who are at risk of or who are already experiencing homelessness in order for them to reintegrate in the community. Major Activities • Provide transportation assistance such as emergency bus tickets, emergency ride assistance (Uber or Lyft), bus passes, emergency gas cards and or clipper cards up to 25 per month • Payment assistance for past due rent with a 3 day notice to evict for up to 3 clie nts per year • Offer 1-3 day motel vouchers for up to 15 clients per year • Unforeseen circumstance situation assistance such as car impoundment (if being used as primary residence), storage lock out, or major car repair for up to 3 clients per year. • One time utility assistance program for up to 5 clients per year • Box lunch station once a week that will provide a pre packed ready to eat meal for up to 15 clients • One time deposit assistance for up to 2 clients yearly (not to exceed $4,000 per year) • Subsidizing rent to low income individuals/families residing within the boundaries of th e City of San Leandro, up to $18,469.08 a year. Clients do not need to be clients of DSFRC to apply. Clients will be assessed and assisted based on need, not by size of their rental unit.
