<br /> <br /> <br />Consulting Services Agreement between City of San Leandro and Last revised 10/28/2021 <br />FVLC for DV Outreach & Mobile Unit Exhibit A – Page 1 of 2 <br />EXHIBIT A <br />SCOPE OF SERVICES <br /> <br />• FVLC, BF, and SAVE will develop an implementation and community outreach plan <br />in coordination with San Leandro city and community-based organization leaders, <br />including youth leaders. <br /> <br />• Monthly meetings with FVLC, BF, and SAVE to coordinate survivor advocacy efforts <br />in San Leandro, led by FVLC. <br /> <br />• Monthly meetings with FVLC Mobile Response Team Manager, Case Manager, SAVE, <br />and SLPD, to develop a referral process for survivors to FVLC’s MRT and discuss <br />ongoing coordination and referral protocols. <br /> <br />• San Leandro community members will have access 24 hours a day, 365 days a year, <br />to FVLC’s Mobile Response Team regardless of whether they contact law <br />enforcement or are comfortable interacting with law enforcement. All of FVLC’s <br />services are confidential and communications with survivors are protected under <br />federal and state law. <br /> <br />• Meetings every other month with FVLC, BF, SAVE, health providers and other <br />providers working with community members experiencing violence and health <br />issues, including CATT, San Leandro Homeless Compact, and the future First <br />Responder Mental Health Team, ACFD, and SLPD to coordinate efforts to support <br />survivors in San Leandro. FVLC also will communicate with these partners outside <br />of these meetings on an as needed basis. <br /> <br />• Case referrals will be made to FVLC who then will handle the matter in house, refer <br />to BF, and/or coordinate with SAVE, depending on the survivors’ needs. <br /> <br />• FVLC’s Case Manager will provide intensive case management to 30 survivors in the <br />first year and BF’s Case Manager will provide intensive case management to 10 <br />survivors in the first year. Both positions together will provide outreach and <br />education, technical assistance and training, and other supportive services that will <br />reach an additional 100 community members. <br /> <br />• FVLC and BF will provide 2 trainings on gender-based violence dynamics and <br />trauma informed approaches to working with survivors for SLPD, 2 trainings for <br />ACFD, 2 trainings for street outreach teams, and a training open to all San Leandro <br />partners. <br /> <br /> <br /> <br /> <br />71
