Final Payments / PROJECT COMPLETION PACKETS <br />OGALS recommends all PROJECT COMPLETION PACKETS be submitted by March 31, 2024. <br />The final payment (20% of the PROJECT amount) will be processed after PROJECT COMPLETION <br />and the following occurs: <br />1. Approval of the PROJECT COMPLETION PACKET and additional required <br />documents in the charts below. <br />2. Site Inspection by the PROJECT OFFICER to verify PROJECT COMPLETION. <br />PROJECT COMPLETION PACKET <br />To request final payment and complete the PROJECT, the GRANTEE must submit the following <br />documents: <br />1. Payment Request Form (pg. 35) <br />2. Grant Expenditure Form (pg. 37) <br />3. Final Funding Sources Form (pg. 14) <br />4. Project Completion Certification Form (pg. 38) <br />5. Completed CEQA, if not already provided (pg. 15) <br />6. Notice of Completion (optional)2 <br />7. Audit Checklist with items marked that the GRANTEE will retain for five years following receipt <br />of final payment (pg. 40) <br />For acquisition PROJECTS, the GRANTEE must submit these additional documents: <br />1. A copy of the recorded deed to the property <br />2. A map sufficient to verify the description of the property including parcel numbers and <br />acreage <br />3. Copy of title insurance policy <br />4. Copy of title report <br />OGALS recommends that the GRANTEE file a "Notice of Completion" with the County Recorder pursuant to State of <br />California Civil Code §3093. Filing the "Notice of Completion" is not a PROJECT COMPLETION requirement. <br />cm <br />
